Connecticut's RHTP Funding: How Remote Care Fits In

The Connecticut Office of Health Strategy (OHS) has released a Notice of Funding Opportunity (NOFO) under the federally funded Rural Health Transformation Program (RHTP), and it represents a meaningful opportunity for eligible providers across the state.


The NOFO is initially making $1.8 million available, with individual awards ranging from $100,000 to $1,000,000. OHS anticipates making up to five grant awards in the first year of the state’s Rural Health Transformation Information Technology Program. Funding is performance-based and available for up to five years. Organizations that demonstrate measurable progress can sustain and grow their programs over time. The application deadline is July 7, 2026. 

What the Connecticut RHTP Grants Are Designed to Fund

The RHTP NOFO is specifically focused on provider-led initiatives that improve healthcare access and outcomes in rural Connecticut communities. The funding opportunity identifies three core clinical objectives:

  • Reducing avoidable hospital utilization
  • Enhancing chronic disease management
  • Strengthening care coordination

These are operational outcomes that sustainable remote care programs are built to deliver.

AI-Enabled Remote Patient Monitoring Is an Explicitly Eligible Use of Funds

The NOFO identifies AI-enabled remote patient monitoring (RPM) as a qualifying use of RHTP grant funding. For eligible providers, this means grant dollars can be directed toward implementing the technology and infrastructure required to monitor patients outside of traditional clinical settings — continuously, at scale, and in a way that generates the outcomes data required for continued funding.

This is a significant detail. Providers evaluating how to structure their applications should understand that RPM is central to the care model the program is designed to support.

Who Is Eligible for CT RHTP Grant Awards?

Eligible lead applicants include hospitals and health systems, federally qualified health centers (FQHCs), independent medical practices, behavioral health providers, municipal and state health departments, academic medical centers, and multi-entity consortia with a clinical lead. Technology vendors may participate as subcontractors.
